I understand and have read the payment rates that are due at the time of services provided. In addition to the “And up”rates depends on the complexities of care for each case, Holiday time, and the time/and a half payment. I also understand that the caregiver’s are paid at the time of service and a check or money order is mail to “NTW” along with the white copy of the time sheets. “Any returned checks will have a $37.00 fee and there will be a $25.00 fee for any late payments.”
